2015 – 2016 NOMINATION FORM

INSTRUCTIONS FOR COMPLETING FORM

 

  • All Nominations must be submitted by the deadline date of MARCH 15, 2016. The Nomination Form will not be available to received additional student nominations after 11:59pm on March 15, 2016.

  • Nominations should be submitted by teachers, school officials, civic organization leaders, etc. Self-nominations from the student or parent / guardian are not accepted. A person can nominate more than one student and more than one student may be nominated from any school.

  • IMPORTANT - To complete the Nomination Form, contact information (i.e., address, phone number(s) and email addresses) will be needed on the student being nominated and/or his or her parent or guardian. If this information is not readily available, it should be collected before starting the input of information into the Nomination Form.

  • Each Nomination Form must be accompanied by a head-and-shoulders photograph of the student that has been selected and provided by the parent or guardian of the student. The photograph can be uploaded as part of the form below.

  • IMPORTANT - In completing the Nomination Form, please provide as much detail as possible about the student and confirm that all parent / guardian contact information (i.e., phone, email address, home address) is correct prior to submitting the form to avoid any delay in the delivery of Award Letters to parents / guardians with the details of any Local Awards Ceremony.
